Time for Data Collection!

Happy New Year!

It is time to complete your Annual Information Request and provide your 2025 plan year census so we can perform required compliance testing and prepare your 2025 Form 5500 package.

To keep your filing and participant notices on track, please return the requested items by Friday, February 13, 2026.

What we need from you (2025 plan year)

Please provide the following items for the period January 1, 2025 through December 31, 2025.

Annual Information Request (AIR)

Census and compensation data

  • Employee level census for everyone eligible during the year, including those who terminated.
  • Required fields include: name, SSN, date of birth, date of hire, rehire date (if applicable), termination date, eligibility date, ownership status, officer status (if tracked), and hours.
  • Compensation definition used by the plan (W-2 Box 1, 3401(a), 415 safe harbor, or other) and the 2025 compensation amount for each employee under that definition.

Contribution and payroll detail

  • 2025 employee deferrals (pre-tax and Roth), catch-up, and loan repayments by pay date and deposit date.
  • 2025 employer contributions (match, nonelective, safe harbor, profit sharing) and dates funded, if already deposited.

Plan events and operational updates

  • Any ownership changes, acquisitions, dispositions, or controlled group or affiliated service group changes.
  • Any plan design or operational changes (eligibility, entry dates, match formula, safe harbor status, auto-enrollment, Roth availability, loan policy changes).
  • Any plan amendments adopted in 2025 (or intended for 2025) and copies of signed amendments (if not already provided).
